Home Safety Program

Our team conducts a thorough assessment of our patients’ homes for safety hazards. We then recommend adjustments as needed to ensure that our patients are able to fully recover safely in their homes.
Some patients have difficulty at home as they transition from rehabilitation in the hospital or nursing home to their own private homes. Their mobility may still be limited and they may require assistance with daily tasks from time to time. Most of all, patients are at risk of falling or losing their balance—incidents that can potentially warrant re-admission. To prevent this, our team provides:

  • Low Vision Safety Program
  • Fall Prevention
  • Posture Correction
  • Physical Therapy

LVAD Care and Teaching

We provide our clients with training and teaching about newly inserted Left Ventricular Assist Device(s) (LVAD) with Circulation Support. We monitor and observe for:

  • Driveline Infection
  • PT/INR Lab
  • Neurological Assessment
  • Post-Heart Surgery Complications
  • Patients with Heart Failure Tendencies

Because patients who need this service are medically-fragile, we offer round-the-clock availability.

Wound Care

We provide our patients with wound care and regular dressing changes to treat wounds, including but not limited to:

  • Soft Tissue Infections
  • Thermal Burns
  • Skin Grafting and Flaps
  • Severe Insect Bites
  • Animal Bites
  • Post-Op Wounds
  • Driveline Site

IV Therapy

IV Therapy is supervised by nurses for patients who need Medication Administration via IV. IV Therapy and Management covers cases involving:

  • Multiple Sclerosis
  • Antibiotic Therapy

Home Health Services

  • Care for clients with recent or new medical diagnosis or disease
  • Exacerbation of an existing condition or disease (like COPD, diabetes, heart disease)
  • Health Management for Progressive deterioration in body function
  • Teaching regarding new and revised medication
  • Urinary catheter care; bladder and bowel training program
  • Nutritional support (new feeding tube or alternate way of feeding/eating)
  • Diabetic care and teaching
  • Recent marked decline in functional status (walking, balance, strength, endurance, ability to care for oneself)
  • Recent falls, fractures, strains
    • Orthopedic Program
    • Total Hip Replacement (THR)
    • Open Reduction Internal Fixation (ORIF)
    • Total Knee Replacement (TKR)
  • Post Stroke Care

  • Skilled Nursing (Includes assessment. planning, implementing, evaluating, and teaching patients as well as monitoring ordered treatments by the physician)

  • Occupational Therapy (restoration and ADL training to reach a better functional level)
  • Speech Therapy (rehabilitation after stroke. head injury and neurological disease)
  • Circulatory Support (assesses the mechanical device – LVAD – and reports any Incidence, complaints and/or equipment malfunction to the physician)
  • Home Health Aides (including assistance with care such as bathing, dressing, grooming, changing bed linens, feeding, toileting, transfers, ambulation, range of motion exercises and light meal preparation)
  • Social Services (social workers to assist in obtaining needed community resources)
